Genesee Health Plan has been awarded funding through the following grant programs to provide services.
Funding awarded to develop a process to identify approximately 200 underinsured women in Genesee County in need of breast or cervical cancer screenings and would benefit from patient
navigation. The program targets women ages 40-64, income levels less than or equal to 250% FPL, and who have barrier(s) that may prevent them from obtaining needed services.
Genesee Health Plan Health Navigation Team will provide patient navigation interventions including barrier identification and resolution to assist the women in receiving breast/cervical screenings and follow up services.
Genesee Health Plan awarded funding to provide chronic disease risk factor screening and healthy lifestyle behavior support to approximately 200 low income women in Genesee County ages 40-64 with no insurance.
