Women’s Health Programs


Genesee Health Plan has been awarded funding through the following grant programs for FY 2017:

Michigan Department of Health and Human Services

Breast and Cervical Cancer Control Navigation Program (BCCCNP)

Funding awarded to develop a process to identify approximately 200 underinsured women in Genesee County in need of breast or cervical cancer screenings and would benefit from patient navigation.  The program targets women ages 40-64, income levels less than or equal to 250% FPL, and who have barrier(s) that may prevent them from obtaining needed services. 

Genesee Health Plan Health Navigation Team will provide patient navigation interventions including barrier identification and resolution to assist the women in receiving breast/cervical screenings and follow up services. 

Wise Woman Program

Genesee Health Plan awarded funding to provide chronic disease risk factor screening and healthy lifestyle behavior support to approximately 200 low income women in Genesee County ages 40-64 with no insurance. 

Genesee Health Plan Health Navigators will:

  • Identify and communicate risk factors for cardiovascular disease, stroke, diabetes and other chronic diseases;
  • Encourage healthy lifestyle choices.
  • Address health equity and social justice issues targeting outreach to underserved populations. 

Program Eligibility

  • Female between the ages of 40-64 years old.
  • Income at 250% FPL or less.

Health Navigators Will:

  • Identify and address barriers/problems to accessing breast/cervical cancer screenings such as:
    • Women who are uninsured or underinsured
    • Lack transportation
    • Experiencing a language barriers and/or
    • Any other barriers preventing them from accessing services
  • Chronic Disease Management
  • Provide individualized support and assistance to overcome identified barriers/problems
