What is Health Navigation?

  • Program available to GHP Members only, that provides personal support for managing long-term diseases and making healthy lifestyle changes. A team of caring Health Navigators who are also registered nurses provide support and connection to community programs.

Health Navigation Program is available for GHP Members only.

How Health Navigators Help:

  • Educate and help with long-term disease management of asthma, diabetes, high blood pressure, high cholesterol, mental health and pain management
  • Nutrition/diet support
  • Quit smoking support
  • Exercise and healthy lifestyle support
  • Obtaining Prescribed Medications that are not covered by your health plan or insurance

What is a Community Health Worker Navigator?

  • Connects you with health services including enrollment and other resources
  • Referrals and connections for food, clothing and other needs
  • Provides utility/energy resources
  • Transportation to medical appointments
  • Breaking down barriers to accessing essential, basic needs and health care
