Front Desk-Enrollment Specialist

Under the general supervision of the Director of Operations, this full-time position will provide primary front desk coverage, provide assistance to the Director of Outreach, provide back-up for the Vision Program, as well as assist individuals with determining their eligibility for the Genesee Health Plan, Healthy Michigan Plan or assist with purchasing insurance on the Michigan Marketplace. If an individual is found eligible, the Enrollment Specialist assists with educating and enrolling them into appropriate coverage. The Front Desk-Enrollment Specialist will interact with the public, providers, Third Party Administrator Customer Service and other Enrollment staff when questions or issues arise. If an individual does not qualify for the Health Plan, the Healthy Michigan Plan or chooses not to purchase insurance on the Michigan Exchange, this position will link the person to community resources and other safety-net providers. Health Educator

This position will support prevention and disease management for Genesee Health Plan (GHP). The Health Educator will perform interventions with GHP patients and providers to promote healthy lifestyles and self-care in patients with and with out chronic disease to ultimately improve health. The Health Educator will place special attention on addressing the social issues that pose barriers to health and healthcare for low-income patients.

Principal Duties and Responsibilities

  1. Building positive relationship with patients
  2. Supporting patients in their behavior change efforts
  3. Assessing their readiness for change and barriers to change
  4. Assisting patients in overcoming barriers
  5. Intervening and following up with patients and their providers in face-to-face, telephone, written, and/or electronic formats
  6. Assessing and addressing the health promotion, chronic disease and social needs of patients through appropriate intervention and/or referral
  7. Completing patient assessments as scheduled (more)

Enrollment Specialist

Under the general supervision of the Director of Operations, this full-time position will assist individuals with determining their eligibility for the Genesee Health Plan (GHP), Michigan Medicaid Programs, the Affordable Care Act (ACA) Insurance Marketplace, Michigan Medicare/Medicaid Assistance Program (MMAP) or GHP Dental Programs. If an individual is found eligible, the Enrollment Specialist will assist with education and enrollment into the appropriate insurance plan(s). The Enrollment Specialist will interact with Third Party Administrator or Delta Dental Customer Service and other Enrollment staff when questions or issues arise. This position may re-enroll GHP members who have been a past member for one year and still qualifies for the Plan, if the individual is not eligible for any other insurance program. If an individual does not qualify for GHP, this position will link the person to community resources, other safety-net providers and other health insurance coverage. (more)


Community Health Worker

Under the direct supervision of the Community Health Worker Coordinator and general supervision of the Director of Operations, this position will assist uninsured individuals with determining their eligibility for Genesee Health Plan benefits, Medicaid, MIChild, the Healthy Michigan Plan (HMP), or by assisting individuals with purchasing insurance on the Michigan Marketplace on an as-needed basis. The Community Health Worker (CHW) will be responsible for helping individuals and their families to navigate and access community services, other resources, and adopt healthy behaviors. This position will also provide health education and assistance to individuals, siblings, parents, and caregivers throughout the community. The Community Health Worker (CHW) may interact with community-based organizations including school staff and administrators, providers, the public, and other GHP Health Navigation and Enrollment staff when questions or issues arise.

The CHW will also conduct assessments/interventions and Community Health Needs Assessment (CHNA) surveys with members for chronic disease management, self-care and prevention. The CHW will place special attention on addressing the social issues that pose barriers to health and healthcare for low-income individuals. (more)
