Community Health Worker – SIM Genesee

Under the direct supervision of the Community Health Worker Coordinator and general supervision of the Director of Operations, this full-time position will assist Medicaid beneficiaries as part of the Michigan Department of Health and Human Services Genesee State Innovation Model which provides for the screening, assessment and referral of target population by clinical healthcare services, behavioral health services and social services to address Social Determinate of Health needs.. The Community Health Worker (CHW) will be responsible for helping identified Genesee County residents with high Emergency Department utilization by providing community linkages in the areas of transportation, housing/utilities, water, basic needs, health coverage, human trafficking, etc.to help promote usage of their Patient Centered Medical Home. The Community Health Worker (CHW) will interact with providers, the public, and other GHP Health Navigation staff when questions or issues arise.

The CHW will place special attention on addressing the social issues that pose barriers to health and healthcare for low-income patients.

Health Navigator – SIM Genesee

Under the general supervision of the Director of Operations, this position will provide health education and assistance to individuals who are enrolled in participating practices in the State of Michigan State Innovation Model (SIM) Genesee program. This position will assess the health status of individuals and identify social determinants of health needs, provide referrals to other community resources to address gaps in the member’s immediate health and social needs. This position supports and promotes access of health care through patient centered medical home This position will place special attention on addressing the social issues that pose barriers to health and healthcare for low-income persons.

This position will interact with the public, medical providers, and other Health Navigation, staff when questions or issues arise. When providing Health Navigation duties, this position will conduct assessments and interventions with GHP members for chronic disease management, self-care and prevention linking the patient to community resources and other safety-net providers.
