The role of the Genesee Health Plan Health Navigation Program is to break down barriers and help patients access community resources for disease management, education and healthy lifestyle changes. We want to partner with you and your doctor to make sure you receive the best care and support.

Evidence shows that patient participation in his/her care plays a key role in self-managing chronic disease. We can serve as an additional resource to help doctors help their patients.

What Health Navigation and what can it do for me?

  • Support and help you manage your chronic disease.
  • Teach you to make healthy lifestyle choices.
  • Break down barriers to care.
  • Work closely with you and your doctor to improve your health.

  • Identify Health Care Needs
  • Disease Education
  • Obtaining Diabetic Monitoring Supplies
  • Obtaining Prescribed Medications through Prescription Assistance Programs
  • Nutrition Support
  • Smoking Cessation Support
  • Exercise Resources

What is the cost?

There is no cost for GHP members to use the Health Navigation and Disease Management services. Some of the programs we refer you to may have fees, but your Health Navigator will inform you of these.
