Genesee Health Plan Community Health Workers (CHWs) began providing services for the Crim Fitness Foundation Community Education Initiative School-Based Program for the Flint Community Schools in May 2015. The goals and objectives of this program are to improve access to health coverage for the uninsured, connect families with a medical home and community resources, health education and eliminate barriers to health care.
