Program Background

Genesee Health Plan Community Health Workers (CHWs) began providing services for the Flint Community Schools in May 2015. The goals and objectives of this program are to improve access to health coverage for the uninsured, connect families with a medical home and community resources, health education and eliminate barriers to health care.  CHWs are here to help all members of the community.  See below for more details and how to contact your Community Health Worker!

In 2024, through a grant received by the State of Michigan, Department of Labor and Economic Opportunity, Genesee Health Plan was able to expand its Community Health Worker Program to include areas in Genesee County, beyond Flint Community Schools and the City of Flint. This project is being supported, in whole or in part, by federal award number SLFRF4951 awarded to The State of Michigan by the U.S. Department of the Treasury.
