Women’s Health Programs


Genesee Health Plan has been awarded funding the Michigan Department of Health and Human Services (MDHHS) to provide the WISEWOMAN program to eligible women. 

WISEWOMAN Program

Program is designed to help women lower their risk for heart disease and/or stroke with healthy lifestyle behavior changes. Women ages 40-64 who meet the income guidelines listed at the bottom of this page are eligible.

GHP Health Navigation Team will:

  • Identify and address barriers/problems to accessing breast and cervical cancer screenings such as:
    • Women who are uninsured or underinsured
    • Lack transportation
    • Experiencing a language barriers and/or
    • Any other barriers preventing them from accessing services
  • Provide individualized support and assistance to overcome identified barriers/problems
  • Address health equity and social justice issues targeting outreach to uninsured and underserved populations
  • Identify and communicate risk factors for cardiovascular disease, stroke, diabetes and other chronic diseases
  • Encourage healthy lifestyle choices
